   If requesting INVITE CARDS and SPECIAL SHAPES ONLY:
   Simply complete Section #1 and Section #4 below.

    INVITE MATCHING MATERIALS:
   If you have a quote or invoice for your Invite Matching Materials DO NOT complete this request form.
   Simply send your quote/invoice via fax to:480-287-9360 or send via email to: sponsorship@gospelbiz.com

   If you need a quote for your Invite Matching Materials, Gospelbiz will obtain one for you.
   Simply complete Section #2 and Section #4 below. We will email the lowest 3rd party quote we can find to
   you for your approval.

Note: Receive 100% Sponsorship when purchasing Standard 3.5" x 2" Invite Cards. You do not have to order the entire 2,500 invite cards at the same time. Example: You can order 1,000* invite cards the first time then change your artwork and order another 1,000* then order 500.* Your 100% Sponsorship is good for any increments up to 2,500 invite cards per Easter Event or Sermon Series. If you need more than 2,500 invite cards, additional cards can be purchased in increments of $20.00 per 1,000. *Shipping charges not included. All invite cards are printed on 14pt card stock.

Terms and Conditions:
When submitting this Invite Sponsorship Request Form, you understand and agree to the following:

INVITE CARDS
Gospelbiz will pay 100% of the cost for any increments up to 2,500 invite cards per Easter event or Sermon series.
You agree to pay $20.00 per 1,000 invite cards over 2,500. UPS Ground shipping charges to the shipping address above ranging from $27-$50 for up to 2,500 invite cards and shipping fees for any increments over 2,500 invite cards will be paid
in full before printing artwork.

SPECIAL SHAPE INVITE CARDS
Gospelbiz will provide 50% in sponsorship dollars towards the cost of the on-line Special Shape Pricing for invite cards.
Your 50% portion and shipping fees for invite cards will be paid in full before printing artwork.
You do not have to use the on-line special shape pricing if you find a quote that is apples to apples and less than the 50% sponsorship price. We will print the special shape invite card for the lesser price and sponsor 100% of the shipping cost.

MATCHING MATERIALS
When submitting section #2 of the form you are saying that you do not have a current quote or invoice for the item you want sponsored. By submitting this form, you are asking Gospelbiz to search and obtain a quote for you.
Gospelbiz will obtain and send you the lowest quote from a 3rd party vendor for you to consider.
If you approve of the quote as a purchase that you can afford and will pay for, Gospelbiz will submit it for Sponsorship Dollars.
After Sponsorship Dollars, your portion and shipping fees for matching materials will be paid in full before printing artwork.
